Coaching Relationship

This agreement is entered into between Amber Moody (“Coach”) and the below-named Client.

Integrative Wellness Coaching is a collaborative process focused on the Client’s current and future priorities, goals, values, and overall well-being. Coaching may address multiple areas of personal growth including but not limited to relationships, career, spirituality, values, finances, education, recreation, health, communication, life transitions, decision-making, emotional wellness, resilience, and stress management.

The purpose of coaching is to support the Client in increasing self-awareness, clarifying goals, creating meaningful change, and developing strategies for personal and professional growth.

Client Responsibilities

As a Client, I understand and agree that:

  1. I am fully responsible for my physical, emotional, mental, and spiritual well-being, including my choices, decisions, actions, and results.

  2. I am responsible for attending scheduled coaching sessions on time, whether conducted via Zoom, telephone, or in person.

  3. I agree to communicate honestly and openly throughout the coaching process.

  4. I understand that successful coaching requires active participation, commitment, and willingness to take action.

  5. I understand that I may discontinue coaching at any time.
     

Mental Health & Professional Services
  1. I understand that Integrative Wellness Coaching is not psychotherapy, counseling, mental health treatment, psychiatric care, substance abuse treatment, legal advice, financial planning, medical care, or any other licensed professional service.

  2. I understand that Amber Moody is not functioning as a licensed mental health professional and that coaching is not intended to replace counseling, psychiatric treatment, medical care, or other professional services.

  3. If I am currently receiving mental health services, psychotherapy, counseling, psychiatric care, or treatment for substance use concerns, I agree to inform my Coach.

  4. If circumstances arise indicating that I may benefit from mental health services or another licensed professional resource, the Coach may recommend referral to an appropriately qualified professional.

  5. I agree to seek professional medical, psychological, legal, financial, or other specialized advice when necessary.

 

Mindfulness, Wellness & Complementary Practices
  1. As part of the coaching process, and when appropriate to the Client’s goals and desired outcomes, sessions may incorporate complementary practices such as mindfulness, guided meditation, breath-work, emotional awareness exercises, visualization techniques, stress-reduction tools, and Reiki energy therapy.

  2. Any such practices will only be introduced with the Client’s knowledge and permission. Participation in any exercise, technique, or practice is entirely voluntary, and the Client may decline or discontinue participation at any time without affecting the coaching relationship.

  3. These approaches are intended to support personal growth, self-awareness, emotional wellness, resilience, and stress management.

  4. The Client understands that mindfulness, wellness, and complementary practices, including Reiki energy therapy, are not intended to diagnose, treat, cure, or prevent any medical, psychological, or psychiatric condition and are not a substitute for licensed healthcare or mental health services.
     

Coaching Process

  3. Accountability

  4. Brainstorming

  5. Values clarification

  6. Written exercises

  7. Education and self-discovery practices

  8. Lifestyle assessment

  9. Communication development

  10. Identifying action plans

  11. Examining habits and behaviors

  12. Challenging limiting beliefs

  13. Exploring opportunities for growth and change

The Client understands that coaching outcomes cannot be guaranteed and that results depend largely upon the Client’s commitment and participation.

 

Confidentiality
  1. The coaching relationship is confidential.

  2. All information shared during coaching sessions will remain confidential except under the following circumstances:

  3. The Client provides written permission for disclosure.

  4. Disclosure is required by law.

  5. There is a reasonable belief of imminent harm to the Client or another person.

  6. There is suspected abuse or neglect where reporting is legally required.

  7. The Coach may discuss coaching cases anonymously with mentors, supervisors, or peer professionals for consultation, training, or professional development purposes while protecting the Client’s identity.
     

Communication Between Sessions
  1. Email and messaging communication between sessions is intended for scheduling, accountability support, brief coaching questions, and administrative matters.

  2. The Coach will generally respond within 24–48 business hours.

  3. The Client understands that coaching communication is not intended for crisis support or emergency situations.
     

Scheduling, Cancellation & Missed Sessions
  1. The Client agrees to provide at least 24 hours notice when canceling or rescheduling a session.

  2. Sessions canceled with less than 24 hours notice, except in cases of emergency, may be considered forfeited and counted toward the coaching package.

  3. Clients are responsible for attending scheduled sessions and arriving on time.
     

Fees & Payment
  1. All coaching fees are payable in advance unless otherwise agreed upon in writing.

  2. Coaching Packages are valid for 12 months from the date of first session

  3. Payments are generally non-refundable once coaching services have commenced unless otherwise agreed in writing.

  4. Additional sessions may be scheduled separately when mutually agreed upon.
     

Privacy & Personal Boundaries
  1. The Client may decline to discuss any topic at any time during a coaching session.

  2. The Coach agrees to respect the Client’s boundaries and personal preferences throughout the coaching relationship.
     

Termination
  1. Either party may terminate the coaching relationship at any time with written notice.

  2. Unused sessions may be rescheduled within six months of the original agreement date in cases of illness, emergency, or other extenuating circumstances, at the Coach’s discretion.
